DESCRIPTION:The Esperanza is conducting a Community survey to find out how you are doing during these times\, to help with referrals for needs we cannot meet\, and to determine what types of programming to offer you. We may not be able to provide you with exactly what you need\, but we will do our best to find you help. We want to gather information about what our community needs to better campaign for our community’s care. \nThe content of this survey is CONFIDENTIAL and intended solely for the use of the Esperanza and of the individual or entity to whom they are addressed. The deadline for the community to submit the survey is May 7\, 2020.
